SPHA FOA Survey Summary

The main purpose of the SPHA program (State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity and Associated Risk Factors program) is to support implementation of cross-cutting strategies to prevent and control chronic diseases and promote health.  The program’s long term goals are to improve prevention and control of hypertension, diabetes and obesity.  To download the full survey summary click here.
