This webinar will explore emerging rural health issues—from chronic disease to access to care to value-based payment—and examine how states and their partners are putting Rural Health Transformation Program investments into action. Participants will see how approaches are taking shape across states and leave with practical tools they can use in their own work, including rural health toolkits, data sources, county-level mortality information, and training opportunities. Together, we will connect timely policy developments with actionable resources to strengthen chronic disease prevention and advance healthier rural communities.
