The CDC 6|18 initiative targets high-burden health conditions with proven interventions through partnerships with health care purchasers, payers, and providers. NACDD collaborates with Leavitt Partners to help states that target diabetes within the 6|18 initiative to gain Medicaid coverage for the National DPP. The states/districts are: Alaska, Georgia, Maryland, Nevada, North Carolina, South Carolina, Utah, and Washington, DC.

Providing support for the 6|18 initiative involved a number of steps, including readiness assessments to determine each state’s propensity and capacity to provide coverage of the National DPP within Medicaid and to identify pertinent topics for technical assistance. Results of these assessments informed in-depth, targeted support for each of the states/districts. A “coverage roadmap”
