What is the NACDD Chronic Disease Certificate Program?
The NACDD Chronic Disease Certificate Program is a training program designed to enhance participants’ understanding of chronic diseases and equip them with the knowledge and skills needed to address opportunities in chronic disease prevention and management.
How long does the certificate program last?
The program lasts 12 months and includes monthly live sessions online.
What is the format of the live sessions?
The live sessions are approximately two hours long and are delivered virtually by subject matter experts.
How much time should I expect to dedicate to the certificate program each month?
In addition to the two-hour live sessions, participants should plan for an estimated four hours per month for additional coursework and activities that emphasize the course concepts covered.
What are the expectations for the Capstone Project?
In the final month, participants will engage in an assessment of a chronic disease project, select an intervention/initiative focus, and present their apporach/strategies that address this focus. The presentation should include an overview of the project, objectives, analysis of existing interventions, evidence-based recommendations, and potential impact and implementation strategies.
What is the level of commitment required from participants for the Chronic Disease Certificate Program?
Participants in the certificate program should be prepared to dedicate time and effort throughout the duration of the certificate program. This includes attendance at monthly live sessions, engagement in coursework/activities, and presenting the Capstone Project. Active engagement and timely completion of coursework/activities are important to gaining the full benefits and successfully achieving a certificate of completion. Participants should contact the certificate program coordinator if they anticipate missing a live session or any coursework/activities.
Will I receive a certificate upon completion?
Yes, participants who successfully complete the certificate program, including any coursework and the Capstone Project, will receive a certificate recognizing their achievement and commitment to addressing chronic diseases.
