Umbrella Hub Arrangements Section of NDPP Toolkit Expands

Over the last several years, the concept of umbrella hub arrangements has gained significant traction as a way of supporting the sustainability of the National Diabetes Prevention Program (National DPP) and other chronic disease prevention and management programs. An umbrella hub arrangement (UHA) is a novel innovation within health care and social services to connect community-based organizations (CBOs) with health care payment systems to pursue sustainable reimbursement for programs such as the National DPP. The Umbrella Hub Arrangements section of the National DPP Coverage Toolkit has quickly become one of the most popular places to visit on the site since it was released earlier this year. This section has recently been expanded to four pages and reorganized to include additional details related to operationalization of a UHA, learnings from the multi-year CDC/NACDD Umbrella Hub Demonstration, and several new resources. The section is now organized into the following pages: UHA Overview, UHA Business Model, UHA Reimbursement, and UHA Sustainability.
