This issue brief summarizes a fireside chat and follow-up virtual roundtables to address team-based approaches to controlling hypertension and highlights pharmacist-related work that from the Connecticut Department of Public Health. During the peer-to-peer virtual roundtables, several states shared their experience in promoting team-based care, including Alaska, Iowa, Nebraska, Nevada, Utah, Vermont, Wisconsin, Washington, and Colorado.
