What is the Cardiac Rehabilitation Change Package (CRCP)?
Cardiac rehabilitation (CR) is a clinical guideline-recommended secondary cardiovascular disease prevention program that includes medically-supervised exercise training, educational and lifestyle modification services (including nutritional and psychosocial counseling), and cardiovascular disease risk factor management. Despite its value in cardiovascular disease prevention, CR is largely underutilized in the United States.
The Million Hearts®/AACVPR Cardiac Rehabilitation Change Package, 2nd Edition (CRCP) offers systems and strategies that can be implemented by quality improvement teams from hospitals and CR programs to increase participation in CR. It is composed of 4 focus areas – Systems Change, Referrals, Enrollment and Participation, and Adherence – with a total of 16 notions of change (or change concepts), 63 specific strategies (or change ideas), and over 200 tools and resources gathered from 40 organizations and agencies to implement the strategies.
Million Hearts® partnered with the National Association of Chronic Disease Directors to develop 4 brief (<20 minute) videos to help quality improvement teams:
Learn about high impact strategies and featured tools from the CRCP
Gain insights from subject matter experts to support implementation of strategies, and
Build capacity and confidence for teams to implement at least 1 new change idea
The videos are posted below to view or share with teams and partners to implement strategies from the CRCP to increase CR participation nationwide.
