Public Health Issue:

  • The Ohio Disability and Health Program implemented NACDD’s Reaching People with Disabilities through Healthy Communities project in two of Ohio’s 88 Counties.
  • To expand inclusion efforts to more communities, the Ohio program needed to create effective partnerships between state and local health officials and to offer technical assistance.

Program Action:

  • Ohio’s Creating Healthy Communities (CHC) program is a state-level program that receives Centers for Disease Control and Prevention State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity and Associated Risk Factors and Promote School Health (DP13 1305) funding to improve community wellness.
  • The CHC local grantees represented ideal partnership candidates for disability inclusion efforts since their community-based work is funded already.
  • The Disability and Health state expert partnered with CHC staff at the Ohio Department of Health to provide technical assistance to 21 more counties in Ohio on promoting disability inclusion.

Impact/Accomplishments:

  • All 23 CHC grantees now incorporate aspects of disability inclusion in their work plans. For example, the CHC county work plans address increased access and opportunity for inclusive physical activity and nutrition.
  • CHC grantees actively promote many of the tools from the NACDD national pilot project, such as the Community Health Inclusion Index and Guidelines, Recommendations, and Adaptations, Including Disability.
  • When the project ends, these efforts will be sustained by the Ohio Department of Health and Ohio’s Disability and Health Program.
