Overview: The National Association of Chronic Disease Directors (NACDD), in coordination with CDC’s Division for Heart Disease and Stroke Prevention, is pleased to announce an opportunity for selected teams to participate in a learning program designed to accelerate team-based care by applying the Identify-Assess-Act Framework. This framework aims to identify and engage with payers, pharmacies, and/or the community to increase pharmacy-based interventions for hypertension control and cholesterol management. To the extent possible, the work from this project will help inform pharmacy-related work in Years 2 through 5 of DP18-1817 (1817).
