This issue brief summarizes the NACDD fireside chat and virtual roundtables with staff from the South Carolina Department of Health and Environmental Control (DHEC) and featured their longtime partner, Dr. Brent Egan, senior medical director of the Care Coordination Institute(CCI). Several themes emerged from the discussions including the role of data in identifying areas or populations with high rates of hypertension; the need for a deliberate intention to address health disparities and achieve health equity when providing technical assistance; and the importance of working with partners who share your mission.
