Umbrella Hub Arrangement Summit

The National Association of Chronic Disease Directors (NACDD) through the support of the Centers for Disease Control and Prevention (CDC) Division of Diabetes Translation (DDT) will host the Umbrella Hub Arrangement (UHA) Summit in Atlanta, September 8 & 9, 2022. A UHA is a novel business arrangement that connects community-based organizations with health care payment systems to pursue sustainable reimbursement for the National Diabetes Prevention Program (National DPP) lifestyle change program.

The UHA Summit is a dissemination of the lessons learned from Umbrella Hub Demonstration Organizations, Learn. Explore. Activate. Problem Solve. (LEAP) participants, Medicaid Beneficiary Enrollment Project (MBEP) participants, Medicare Diabetes Prevention Program (MDPP) Enrollment Project participants, state health departments, national organizations, and other partners. This 1.5-day summit will convene emerging UHAs for peer-to-peer discussion and sharing of successful strategies to operationalize a UHA.

The “Building the Delivery Infrastructure for the National DPP Through Strategic Partner Convenings” project is supported by the Centers for Disease Control and Prevention of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $900,000 with 100 percent funded by CDC/HHS.
