Members of the CVH Team have led six funding recipients through a Million Hearts Health Equity Implementation from January through June 2023. Organizations were selected to implement evidence-based strategies to improve cardiovascular health and advance health equity. In this celebration meeting, funding recipients will share their experience, successes and challenges implementing strategies outlined in Million Hearts® 2027 Focusing on Health Equity among priority populations. This is a one-time meeting that will include representatives from CDC, NACDD, funding recipients, and CVH staff of SHDs corresponding to the funding recipients.
