Umbrella hub arrangements (UHAs) connect community-based organizations with health care payment systems for the National DPP and other evidence-based programs. In their work to form a UHA, the Minnesota Department of Health has partnered with Trellis, a community service organization that provides services, information, and innovations to achieve their mission of assisting individuals to age well and developing the capacity of communities to care for an aging population. Trellis serves as an umbrella hub organization (UHO), which is the lead entity in a UHA. Since the launch of the Minnesota National DPP UHA in 2021, Trellis has become a Medicaid enrolled provider in the state and a Medicare DPP (MDPP) supplier; it has also secured contracts with Medicaid managed care organizations and recruited nine subsidiary organizations into their UHA.
The Trellis UHA embedded the Centers for Medicare and Medicaid Services (CMS) Accountable Health Communities Health-Related Social Needs Screening tool in its secure, web-based management information system (www.yourjuniper.org) to allow subsidiary organizations to better support participant engagement and retention in the lifestyle change program. In addition, the Trellis tool maps the screening results to Z codes, allowing organizations to collect diagnostic criteria for health related social needs.
