As the National Diabetes Prevention Program (National DPP) Coverage Toolkit approaches its tenth year, NACDD is reflecting on its legacy, growth, and continued evolution. What began as a simple repository for information and lessons learned related to Medicaid coverage of the National DPP has grown into a comprehensive, one-stop shop to support users “from policy to payment.” Today, the Toolkit provides easy access to tools and resources that support sustainable chronic disease prevention and management programs across communities, states, and regions.
The Coverage Toolkit was launched in June 2017 by the Centers for Disease Control and Prevention (CDC) and NACDD as part of the Medicaid Coverage for the National DPP Demonstration Project. It was developed with input from a wide range of partners, including health plans, national organizations, state Medicaid agencies, State Health Departments, and federal stakeholders. Since then, the Toolkit has expanded to include content on Medicaid and Medicare policy and programming, Umbrella Hub Arrangements (UHA), and Community Care Hubs (CCH). It also features resources on engaging managed care organizations and value-based care, partnering with pharmacists, Federally Qualified Health Centers (FQHC), and correctional facilities, and meeting the unique needs of program participants with gestational diabetes and people with disabilities.
More recently, the Toolkit has broadened its scope to reflect the growing integration of chronic disease prevention and management efforts. New content includes information on diabetes self-management education and support (DSMES), Food is Medicine initiatives, and Family Healthy Weight Programs.
The Coverage Toolkit attracts a diverse and growing audience—now exceeding 220,000 users—including representatives from state and local health departments, federal agencies, Medicaid agencies, managed and accountable care organizations, commercial and employer health plans, health systems, and program delivery organizations. NACDD’s extensive diabetes project portfolio, cross-cutting work within the association, and strong partnerships at the local, state, and federal levels contribute real-world insights and examples that strengthen and inform the Toolkit’s content.
