NACDD’s partnership with the Council of State and Territorial Epidemiologists (CSTE) has resulted in the release of the new Chronic Disease Surveillance Modernization Toolkit—a flexible, web-based resource designed to support state, Tribal, local, and territorial public health agencies as they strengthen and modernize their chronic disease surveillance systems. The toolkit aligns with the National Strategic Plan for Chronic Disease Surveillance Modernization and includes implementation guides, templates, success stories, training resources, and more. 

The toolkit was developed with extensive input from STLT partners and offers practical, modular resources tailored to agencies at different stages of modernization.
