The National Association of Chronic Disease Directors (NACDD) is collaborating with Association of State and Territorial Health Officials (ASTHO) and the Centers for Disease Control and Prevention (CDC) on two projects that will provide support to U.S. territories and freely associated states on type 2 diabetes prevention and management efforts.
These projects support U.S. territories and freely associated states in the Pacific (American Samoa, the Commonwealth of the Northern Mariana Islands, the Federated States of Micronesia, Guam, the Republic of the Marshall Islands, and the Republic of Palau) and Caribbean region (Puerto Rico and the U.S. Virgin Islands), offering tailored technical assistance, training, and peer-learning opportunities based on each regionās goals and local context. Regional subject matter experts with understanding of local health systems and context and regional organizations guide the development and delivery of trainings and technical assistance to support. Since 2011, NACDD has partnered with CDC, national public health organizations, and state health departments/ministries of health in the territories and freely associated states to support diabetes quality improvement initiatives in local community health centers through the Noncommunicable Disease (NCD) Collaborative. NACDD will continue this work and collaborate with NCD Collaborative teams to determine priorities over the next five years and support the NCD Collaborative with technical assistance and training.
NACDD will provide technical assistance and support on diabetes public health strategies for eight recipients from the Pacific and Caribbean U.S. territories and freely associated states funded under a chronic disease cooperative agreement from CDC. Assistance will be delivered through a variety of formats including trainings to facilitate skill building, program development, peer learning, and partnership engagement through data-driven planning processes related to diabetes prevention and management.
These combined efforts seek to ensure that U.S. territories and freely associated states have the tools, support, and local guidance they need to respond to the rising burden of type 2 diabetes and other chronic diseases.
