NACDD supports the recipients of CDC’s National Asthma Control Program (NACP) through its National Asthma Control Program Collaborative. Launched in 2023, this collaborative brings NACP recipients together to share promising practices, explore health information approaches, and find creative ways to integrate health equity into their EXHALE strategies.

The support we provide to NACP recipients focuses on:

  1. Providing an avenue for recipients and national partners to leverage each other’s expertise to enhance asthma programming and services.
  2. Exploring and learning how electronic health records can support disease surveillance and management.

Advancing Health Equity Through EXHALE Strategies

By promoting social justice and wellbeing, communities can build healthier futures. NACDD’s Social Justice Framework can support asthma program teams to center health equity within their EXHALE work.

When asthma program teams approach each EXHALE Strategy through the lens of the Social Justice Framework’s Five Pillars, they may see a shift in the strength of their partnerships, how they prioritize the communities they serve, which may increase access to care, provide more equitable care, and lead to better control of asthma.

Stories from the Field

A core component of the NACP Collaborative is learning from one another. NACDD invites you to share how you are centering health equity within your EXHALE work. Join our virtual Asthma Community to access archives from the 2024 Asthma Forums, connect with peers, and share your experiences.

As a Member of NACDD, you can also submit stories to our What’s Working in Chronic Disease Prevention and Control database. This is a great way to share your successes with leaders, policy makers, and other chronic disease professionals across the nation.
