NACDD Remains Committed To Inclusion Through 1705 Project and New Website

NACDD was one of 10 recipients of the six-year CDC cooperative agreement, Scaling the National Diabetes Prevention Program in Underserved Areas (DP17-1705). Our project focus was reaching people with disabilities with an inclusive evidence-based intervention, the National Diabetes Prevention Program (National DPP) lifestyle change program (LCP). This project demonstrated the power of relationships as NACDD leveraged national, state, and local partnerships to provide comprehensive technical assistance (TA) and support to affiliate sites across five states (AK, FL, IA, NY, and PA) (pictured below).

Over the six-year project period, NACDD:

  • Worked across five states to support 24 programs in applying for and becoming CDC-recognized to offer the National DPP LCP
  • Supported the enrollment of over 1,400 participants with prediabetes into the National DPP LCP. Twelve percent of participants reported having a disability and achieved an average weight loss of 9.7% (5.2% national average)
  • Supported four sites in becoming Medicare DPP providers as well as four sites in becoming Medicaid suppliers (NY and PA only)
  • Trained and onboarded 12 CDC-recognized organizations onto the HALT platform, resulting in 10 cohorts with 140 participants
  • Provided at least 10 lifestyle coach trainings to 50+ lifestyle coaches, with a focus on inclusion for people with disabilities
  • Provided regular training and technical assistance and one-on-one support to the five states and 20+ recognized programs to support successful implementation of work plans with a focus on inclusion for people with disabilities
  • Created multiple resources and toolkits for states and sites to use in reaching and enrolling people with disabilities into the National DPP LCP (training, intake form, and data collection support; marketing materials; and partnership engagement tools) 

 

A Snapshot of 1705 Successes

NACDD and Lakeshore offered a TA opportunity to support sustainability through mini-grants. These grants aimed to increase 1705 state affiliate capacity to be accessible and inclusive to adult National DPP LCP participants with physical and/or sensory disabilities. As recipients of these grants, Conemaugh Diabetes Institute (PA), Palm Beach County Health Department (FL), and Iowa Community Hub (IA) purchased, stored, and distributed wheelchair-accessible scales; developed an inclusive equipment resource catalog; and fostered partnerships with local rehabilitation providers and Arc chapters (community-based organizations advocating for and with people with intellectual and developmental disabilities and serving them and their families) to recruit and enroll people with disabilities.  

NACDD partnered with the Alaska Department of Health and InquisitHealth to capture a success story video of Allen, an InquistHealth telephonic National DPP LCP participant from rural Alaska who completed the program. Allen was referred to the program by his healthcare provider after experiencing two heart attacks and receiving bloodwork results in the prediabetes range. Through the program, Allen lost almost 60 pounds and can now do the things he loves like hunting, fishing, and swimming with his grandkids with a lot less stress and fatigue. Allen’s story was utilized by the Alaska Department of Health in their ‘Fresh Start’ public health campaign

NACDD partnered with the ProVention Health Foundation to offer the Health And Lifestyle Training (HALT) Diabetes software platform to partner states, enabling any CDC-recognized organization to offer the National DPP LCP online. The HALT platform empowers CDC-recognized organizations to expand network capacity and affordable access to the National DPP LCP. NACDD provides users with onboarding training, recruitment and enrollment support, and customized marketing materials. Over the course of the project, NACDD trained and onboarded 14 CDC-recognized organizations onto the HALT platform, resulting in the enrollment of 245 participants. Learn more about HALT.

Beyond 1705

NACDD is incredibly proud of this project and the strides made towards making chronic disease prevention programs inclusive. We remain committed to inclusion and encourage all chronic disease prevention program providers to utilize Lakeshore Foundation’s Roadmap 2 Inclusion (pictured below) to ensure program inclusivity.
