ATLANTA (Dec. 9, 2022) – The National Association of Chronic Disease Directors (NACDD) is excited to announce recipients of the Million Hearts® Health Equity Implementation funding!
NACDD reviewed more than 100 applications from organizations who submitted proposals in response to the Million Hearts® Health Equity Implementation RFP. The following six organizations were selected to implement evidence-based strategies to improve cardiovascular health and advance health equity:
- Huddle Up Moms (VA): focusing on pregnant and postpartum women with hypertension by providing self-measured blood pressure cuffs and care navigation.
- Midvale Community Building Community (UT): focusing on low-income patients served in their free and charitable care clinic, proving needed blood pressure cuffs and medications needed for tobacco cessation.
- St. James Healthcare (MT): focusing on addressing care desserts for rural heart failure patients.
- UCONN Health Maternal-Fetal Medicine (CT): focusing on pregnant and postpartum women with hypertension by providing self-measured blood pressure cuffs and needed support for at-risk pregnant women.
- University of Pittsburgh School of Pharmacy and Pennsylvania Pharmacists Care Network (PA): focusing on low-income patients with hypertension through a network of community pharmacists. to support patients with hypertension.
- West Virginia University School of Pharmacy (WV): focusing on rural patients by increasing access to care through a pharmacy intervention on virtual medication optimization for heart failure patients.
Each organization will receive up to $50,000 to effectively implement Million Hearts® strategies among priority populations to advance health equity between January – June 2023. Each grantee will focus on a Million Hearts® priority population and implementing appropriate strategies outlined in Million Hearts® 2027 Focusing on Health Equity to address their needs.
