Since 2018, eligible Medicare beneficiaries have had access to diabetes prevention services through the Medicare Diabetes Prevention Program (MDPP). CDC funds NACDD across various initiatives to support access to, utilization, and sustainability of the MDPP benefit. NACDD has created a webpage and downloadable PDF infographic to describe this multi-faceted work, which includes the MDPP Enrollment Project, the MDPP Supplier Learn. Explore. Activate. Problem-Solve (LEAP) series, 1705 MDPP Learning Collaboratives, Rock Enroll LEAP, and MDPP Supplier Learning Series.
Through the MDPP Enrollment Project, NACDD provides individualized and group-based technical assistance, in addition to funding, to program suppliers to increase beneficiary enrollment and help them successfully submit reimbursement claims. In the MDPP Supplier Learn. Explore. Activate. Problem-Solve (LEAP) series and 1705 MDPP Learning Collaboratives, NACDD and CDC subject matter experts help prepare State Health Department (SHD) staff and national 1705 organizational staff to offer technical assistance to CDC-recognized organizations throughout the process of enrolling as MDPP suppliers. The Rock Enroll LEAP initiative supports SHD working with MDPP suppliers as they increase beneficiary enrollment and sustainability in their state. Finally, the MDPP Supplier Learning Series (SLS) convenes suppliers, SHDs, and 1705 organizations. Sponsored by CMS/CDC/NACDD, this year’s MDPP SLS is focused on learning about innovative strategies from SMEs to improve enrollment of Medicare beneficiaries from populations of focus, such as African American, Asian American, and Latina/Latino communities.
