Public Health Issue:To reduce the risk of heart attack and stroke, it is critical for persons with cardiovascular disease (CVD) to have effective structures for medication adherence, proper nutrition and physical activity. This can be especially challenging for low-income and under-served populations such as WISEWOMAN clients.
Approximately 85% of women screened in the WISEWOMAN Program have CVD risks but most have difficulty accessing the necessary resources to manage their health.
Project Objectives:Community Health Workers (CHWs) will link WISEWOMAN patients to community support services.
Program Action:This project targets low-income and under-served populations such as WISEWOMAN clients. Building on the promising use of Community Health Workers (CHW), the Wisconsin HDSP will assist two clinics in integrating CHWs to link WISEWOMAN patients to community support services. Approximately 85% of women screened in the WISEWOMAN Program have CVD risks, but most have difficulty accessing the necessary resources to manage their health.(CHWs) will be trained to help WWWP clients access local resources to support recommended lifestyle modification. Possible evaluation measures are proportion of WISEWOMAN clients who meet with a CHW and a) have a self-management goal, b) take action on modifying their lifestyle, c) adhere to prescribed therapy and d) demonstrate improvement of CVD risks. Next Steps:Will recruit one more FQHC in Green Bay area, secure MOU, and complete protocol and have materials in place.
