About the National Association of Chronic Disease Directors:
The National Association of Chronic Disease Directors improves the health of the public by strengthening state-based leadership and expertise for chronic disease prevention and control in states and at the national level. Founded in 1988, NACDD aims to lead and influence the ways that chronic disease prevention and health promotion shapes the health landscape.
Brief Project Overview:
This project will serve as a new component in Year Five of the five-year NACDD project “Diabetes Technical Assistance and Support for State Health Departments,” which is supported by the Centers for Disease Control and Prevention (CDC) of the U.S. Department of Health and Human Services (HHS). The financial assistance of this award totals $6,486,029 with 100 percent funded by CDC/HHS. *
This project is designed to implement a chronic kidney disease (CKD) detection and education program in a primary care clinic setting using simple tests, routine questionnaires, and patient education. The project must utilize systems change and quality improvement approaches and is intended to build on existing work within primary clinic settings.
Additionally, the projects aim to increase the use of clinical decision support (CDS) within an Electronic Health Record (EHR) to promote early detection of chronic kidney disease (CKD) and education on how to reduce risk factors. The target population will include all individuals who are at least 18 years old and present to the screening with at least ONE of the following:
- Self-reported hypertension and/or diabetes.
- Be 50 years of age or older to qualify.

Eligible Applicants:
- Applicants should have an established partnership with a primary healthcare system that reaches a minimum of 100,000 patients or serve as a healthcare system that reaches a minimum of 100,000 patients
- If applicant is not a health system, applicants must submit a Memorandum of Understanding (MOU) with a primary care health system
- Applicants must be able to initiate the work immediately upon selection
- Applicants must be in “good standing” with the federal government as the dispersed award are federal grant funds
