2019 Million Hearts® Hypertension Control Champions

This year, CDC and the Million Hearts® are recognizing 17 Champions from across the United States. Together, these dedicated professionals cared for nearly 360,000 adults where blood pressure control was achieved in 80% or more of their patients. These remarkable efforts are critical to preventing and managing heart disease and stroke, two leading causes of death for Americans.

The 2019 Million Hearts® Hypertension Control Champions are:

In the seven years since the start of the Hypertension Control Challenge, Million Hearts® and CDC have recognized 118 Million Hearts® Hypertension Control Champions from 36 states and Washington, D.C., serving more than 15 million adults. The 2019 Champions include solo practitioners, large health systems, community health centers, and specialty groups in urban and rural settings. Together, they show that focus and sustained actions can lead to system-wide impact on blood pressure control—all of which translate to heart attacks and strokes prevented and lives saved.
 
Saving lives through better blood pressure control has been a longstanding priority for the CDC and the Million Hearts® initiative. By recognizing the Champions’ performance and sharing their lessons learned, CDC aims to help other health care professionals achieve the same success in communities nationwide.
