Improving the CDC Healthcare Systems Scorecard

Join a 20-minute coffee break for an introduction to CDC’s Healthcare Systems Scorecard (HSSC) tool and rapid evaluation findings. Learn about improvements made to the Scorecard that have enhanced its usability and applicability to cooperative agreements such as 1815 and 1817.
CDC developed a quality improvement assessment tool called the Health Systems Scorecard to help CDC-funded state and local public health programs better understand the implementation of evidence-based strategies to manage chronic diseases among partnering primary care practices. The Scorecard was released to partners for voluntary use in June 2017 and a rapid evaluation was conducted 6 months later. Based on findings from the evaluation, CDC partnered with recipients that implement health system assessments to make improvements to the Scorecard. The presenter will provide a brief introduction to the tool and the rapid evaluation findings, and will summarize the improvements made to the Scorecard to enhance usability, applicability to cooperative agreements such as 1815 and 1817, and enhanced dissemination strategies. After attending this Coffee Break, participants will 1) know how to access the Health Systems Scorecard, 2) gain tips on how to best engage health systems to complete the Scorecard questions, and 3) discover ways to utilize the Scorecard and supplemental materials to report on CDC required heart disease-related performance measures.
