Lincoln Community Health Center is a community-based clinic system in Oregon. It operates two main clinics in Lincoln City and Newport, Oregon, and operates four school-based health centers in high schools. The Lincoln Community Health Center has conducted a mailed FIT program since 2019. This video tells the story of the creation and operation of the first three years of the mailed FIT program.
AltaMed is a large, federally qualified health center system in Southern California. It has more than 30 clinics and serves a predominantly Latino, Spanish-speaking population. AltaMed provides general primary care, dental, and behavioral health services. This video tells the story of the AltaMed mailed FIT program between 2015 and 2022.
The SCORE project was a partnership between the Lineberger Comprehensive Cancer Center at the University of North Carolina at Chapel Hill and two community health centers that serve rural North Carolina communities. The goal of the SCORE project was to reduce colorectal cancer mortality rates by increasing the screening rates among patients in the two community health centers (UNC Example Detailed Workflow).
