LEAP (Learn. Explore. Activate. Problem-solve.) Learning Labs are topic-specific group-based learning opportunities offered through a collaboration between the Centers for Disease Control and Prevention (CDC) and the National Association of Chronic Disease Directors (NACDD) to help State Health Departments actively pursue new or enhanced approaches for scaling and sustaining the National Diabetes Prevention Program. LEAP Learning Lab participants learn and contribute to identifying promising practices, effective tools, and resources. Previous LEAP Learning Labs include Umbrella Hub Arrangements, Rock Enroll into Medicare Diabetes Prevention Program, and Medicare Diabetes Prevention Program Suppliers.
