NACDD is working with state teams on a learning program that is designed to accelerate team-based care using the pharmacists’ patient care process (PPCP) and collaborative practice agreements (CPA) designed to manage high blood pressure. These meetings note summarize a workshop held for the partner teams on May 24-25, 2017 to develope a project plan and timeline for disseminating and supporting pharmacists with implementing the PPCP and CPA resource guides provided by CDC.
