Impact Brief June 2021
The NACDD CVH Team, along with the CDC Division for Heart Disease and Stroke Prevention, hosted a webinar on April 15 featuring a panel of state participants from the Advancing Pharmacy-Related Interventions to Control Hypertension and Manage Cholesterol Learning Collaborative.
Representatives from the Colorado, Georgia, Missouri, Texas, and Virginia state teams shared innovative team-based approaches to chronic disease prevention and management through collaborations including schools of pharmacy, state pharmacy associations, community pharmacists, and a state quality improvement organization. Panelists shared their experiences related to the impact of COVID-19 on the initiative, successful strategies, resources, and opportunities for future collaboration.
