Teri Elkins, MPH, CHES is a Public Health Consultant with NACDD on the Diabetes Team – Umbrella Hub Demonstration Project with a focus on building program sustainability for the National Diabetes Prevention Program (NDPP). Prior to joining NACDD, Teri was the Population Health Programs Director at Sun Health and was responsible for overseeing Sun Health’s community-based Diabetes Self-Management Education and Support (DSMES) program, National Diabetes Prevention Program (NDPP), and Medicare Diabetes Prevention Program (MDPP). Teri has experience with virtual delivery of the National DPP, credentialing, contracting, reimbursement, and strategic planning for the National DPP and DSMES programs.

Teri obtained her Master of Public Health and Bachelor of Science in Health and Wellness from Purdue University Global and is a Certified Health Education Specialists. Teri serves as Chair of the Arizona Diabetes Coalition and sits on the Association of Diabetes Care & Education Specialists Practice Management, Payment, and Reimbursement Committee. She also has more than 20 years of experience in the banking industry. 

Teri has a deep passion for diabetes and overall health and wellness that was inspired when her son was diagnosed with type 1 diabetes in 2007. She resides in Phoenix, Arizona.
