The Centers for Disease Control (CDC) funded the National Association of Chronic Disease Directors (NACDD) to implement the Medicaid Demonstration Project in Maryland and Oregon from July 2016-January 2019. The focus of the Medicaid Demonstration Project was to determine how state Medicaid agencies, in collaboration with State Health Departments, can implement delivery models for the National Diabetes Prevention Program (National DPP) lifestyle change program for Medicaid beneficiaries at high risk for type 2 diabetes through managed care or accountable care organizations. The evaluation of the Medicaid Demonstration Project culminated in the development of two best practices documents presenting overviews of results related to recruitment and retention of participants. These summaries include participant outcome and survey results as well as lessons learned by organizations to help support recruitment and retention of Medicaid beneficiaries in the National DPP lifestyle change program.
