New in NACDD’s Online Learning Center, Building Healthcare Partnerships: Community-Clinical Linkages for Chronic Disease is an introductory course designed for staff in city, county, state, and territorial health departments, as well as community-based organizations. Developed collaboratively by programs in the Center for Advancing Healthy Communities, this self-paced course provides practical strategies to strengthen partnerships between public health and healthcare and better connect community-based programs with clinical care.

The course can be completed in less than two hours and includes four interactive modules that explore how healthcare organizations operate, strategies for building and sustaining healthcare partnerships, effective referral processes, and how healthcare organizations measure success.

Learners will engage with interactive activities, scenario-based knowledge checks, and practical resources that support real-world application. Participants who successfully complete all four modules will automatically receive a Certificate of Completion.
