Jake Sneed is a Program Evaluator for the National Association of Chronic Disease Directors (NACDD), where he provides evaluation services for the Center for Public Health Leadership. With a decade of public health experience, Jake began his career working for state health agencies in South Carolina and Georgia, focusing on cooperative grants aimed at developing and evaluating chronic disease programs.

Jake’s most recent role involved working with the CDC’s Division of Adolescent and School Health, where he evaluated demonstration projects and translated findings into actionable health recommendations. In addition to his professional work, Jake serves in the United States Marine Corps Reserves as a Weapons Platoon Sergeant.

Jake holds a master’s degree in public health from Nova Southeastern University and a bachelor’s degree in integrative health science from Stetson University. He resides in Atlanta with his wife and daughter. In his free time, he enjoys running and spending time outdoors with his family.
