Pharmacy Related Resources

In coordination with CDC’s Division for Heart Disease and Stroke Prevention, NACDD implemented two Learning Collaboratives bringing together state teams of public health and pharmacy partners to accelerate team-based care to manage high blood pressure and high cholesterol. As a result of the two Learning Collaboratives, participant teams now serve as facilitators within their states to continue and enhance successful strategies and share resources.

Advancing Pharmacy-Related Interventions to Control Hypertension and Manage Cholesterol

NACDD and CDC worked with Colorado, Georgia, Missouri, Texas, and Virginia state teams as part of the Advancing Pharmacy-Related Interventions to Control Hypertension and Manage Cholesterol Learning Collaborative. Each participating team was required to have one State Health Department representative and at least one pharmacy partner.

The Learning Collaborative was developed in response to State Health Department interest in working with pharmacy partners (e.g., Schools of Pharmacy, state pharmacy associations, pharmacy chains, and community pharmacists) to meet CDC Cooperative Agreement DP18-1817 priorities focused on innovative team-based approaches to hypertension control and cholesterol management.

The onset of the COVID-19 pandemic early in the initiative disrupted in-person activities, necessitating a redesign of some efforts to ensure the safety of participants. Despite a significant impact of the COVID-19 pandemic on the Learning Collaborative, state teams continued to collaborate and tailor activities to improve access to pharmacist-led services. Between February and December 2020, participants engaged in peer sharing calls; a 1.5-day virtual workshop with subject matter experts on the interventions to share challenges and opportunities; and a one-day closing workshop in December to highlight lessons learned.

Advancing Team-Based Care Through the Use of Collaborative Practice Agreements and Using the Pharmacists’ Patient Care Process to Manage High Blood Pressure


NACDD, in coordination with CDC’s Division for Heart Disease and Stroke Prevention, worked with Arizona, Georgia, Iowa, Utah, Virginia, West Virginia and Wyoming state teams on a Learning Collaborative to accelerate team-based care using the pharmacists’ patient care process (PPCP) and collaborative practice agreements (CPA) designed to manage high blood pressure. State Health Department and pharmacy partner teams attended an in-person workshop and participated in several webinars to acquire needed information to host trainings within their states to support pharmacists in implementing CDC’s PPCP and CPA resource guides.
