National Diabetes Prevention Program Releases 2018 State Engagement Model Collective Impact Report

 

The National Association of Chronic Disease Directors (NACDD) has released the National Diabetes Prevention Program State Engagement Model Collective Impact Report. The work highlighted in this report builds on the efforts and investment of many organizations across the U.S. aiming to reduce the prevalence of prediabetes and type 2 diabetes.

The report highlights the contributions of the NACDD/CDC State Engagement Model that was designed to enhance stakeholder engagement and catalyze commitment for collective action to advance the CDC-led National Diabetes Prevention Program. NACDD is grateful to Leavitt Partners for their expertise and partnership in the creation of this report.

 

State Health Departments and national leaders contributed to the successful progress, outcomes, and impacts summarized in this report. Their continued efforts are critical to engaging public and private partners to implement state and organizational National DPP action plans, many of which began at one of the 31 State Engagement Meetings (SteM) convened between 2012 and 2018.

The report provides detailed Outcome Briefs that:

  • showcase key outcomes resulting from StEM, action plans, and partner engagement activities;
  • share successes, best or promising practices, and lessons learned in implementing action plans and partner engagement activities that contributed to key outcomes; and
  • share methods, tools, and resources for overcoming barriers to scaling up and sustaining the National Diabetes Prevention Program in collaboration with multi-sector partners.

 

As NACDD begins the next phase of work in diabetes management and prevention of type 2 diabetes, the lessons learned adapting NACDD/CDC State Engagement Model for each state, as included in this report, will inform the Association’s support for additional states in the future.
