Million Hearts® is a national initiative to prevent 1 million heart attacks and strokes in the U.S. co-led within DHHS by the Centers for Disease Control and Prevention (CDC) and Centers for Medicare & Medicaid Services (CMS). Questions and Answers

The CVD network hosted a call with Dr. Janet Wright, who provided an overview of Million Hearts® 2022 and discussed some of the roles of state health departments. A critical role is partnering: collaborating with and convening partners. Utah reported on the state efforts to improve clinical care through blood pressure management within its health clinics. Indiana discussed the state’s efforts to use evidence-based clinical protocols within its Eskenazi Health system. Both are excellent examples of the work happening in the field and demonstrates how these Million Hearts efforts and targeted 1305/1422 funded strategies serve to both leverage and synergize the collective work of the Division for Heart Disease and Prevention.

Blog Post: NACDD’s cardiovascular health initiative works with state chronic disease programs on identifying health disparities in hypertension control and scaling up blood pressure control activities in clinical practices. As follow-up to a fireside chat addressing health disparities, we shared information in a blog post for ASTHO’s Heart Disease and Stroke Prevention State Learning Collaborative. ASTHO reached out to its national partners to learn how each organization addresses decision-making, policy opportunities, and skill-building for today’s public health workforce. Publications:

Million Hearts® Stakeholders Workshops: With support from CDC and the states, NACDD worked with 16 state public health agencies beginning in 2012 to plan and facilitate Million Hearts® Stakeholders Workshops and provide follow up technical assistance. Engaging Partners for Million Hearts® Success summarizes the work-shop process and highlights efforts in Iowa, New Mexico, Michigan, and Texas.
Medication Adherence Series: The National Association of Chronic Disease Directors (NACDD) and the Association of State and Territorial Health Officials (ASTHO) implemented two national webinars, each followed by a virtual roundtable call on the topic of helping clinical providers improve the percentage of patients who take their hypertension medication as prescribed. The Medication Adherence Call Series Summary summarizes the series content and provides an overview of actions states are taking. Additional guidance on targeting state actions is found in the Million Hearts® State Engagement Guide.


Undiagnosed Hypertension
To support the Million Hearts focus of identifying people with undiagnosed – and therefore untreated – hypertension, NACDD and the CDC Division for Heart Disease and Stroke Prevention launched a series of learning opportunities in February 2016, including a fireside chat live-streamed from CDC, two virtual roundtables to allow grantees to discuss details of their work related to undiagnosed hypertension, and the development of success stories featuring strategies implemented by state health departments. An issue brief highlights lessons learned and success stories shared by New York, Florida, and Montana about their work related to undiagnosed hypertension. A virtual roundtable document summarizes highlights from the fireside chat and the roundtables and case studies from Colorado, Maine,
Massachusetts, Montana, Texas and Wisconsin.

2018 Million Hearts Hypertension Control Challenge
Many practices and systems work with their patients to achieve blood pressure control. The 2018 Million Hearts® Hypertension Control Challenge will identify clinicians, clinical practices, and health systems that have exceptional rates of hypertension control. Million Hearts® is accepting applications for the 2018 Hypertension Control Challenge from February 20, 2018, through April 6, 2018. More information about the 2016 challenge here.
