The document also asks readers to provide information relating to existing TPOs in their state.

National DPP Coverage Toolkit (2021)
This 1-page fact sheet describes the online National DPP Coverage Toolkit, launched in 2017 by NACDD and Leavitt Partners and funded by CDC, that can help organizations navigate the potential complexities of offering the program as a covered benefit.

Demonstration Project – Medicaid Coverage for the National DPP (2021)
This 1-page fact sheet describes the Medicaid Demonstration Project, funded by CDC, that’s showing how state Medicaid agencies and state health departments can collaborate to implement and deliver a sustainable coverage model for the National DPP.

National Diabetes Prevention Program, State Engagement Model Collective Impact Report
This NACDD report showcases the impressive results of State Health Department’s work with their partners to expand the National Diabetes Prevention Program. The report contains aggregate data collected with a survey as well as State Outcome Briefs (below) that describe efforts to expand the National Diabetes Prevention Program by increasing: 1) Awareness, 2) Availability, 3) Screening, Testing, and Referrals, and 4) Coverage. The Executive Brief includes highlights and summarizes the NACDD/CDC State Engagement Model.

 

Working Together to Prevent Type 2 Diabetes:The National Diabetes Prevention Program State Outcome Briefs
These outcome briefs were submitted by each state and NACDD partner for the purpose of providing information to participants during the National DPP State Engagement Outcomes Summit, held in February 2018. Following the Summit, a few states submitted new or revised briefs and these are included along with all briefs from the original Summit participant working copy of the document.

Nebraska Diabetes Prevention Action Plan 2016 – 2018
The Nebraska Department of Health and Human Services finalized this action plan just four months after hosting a State Engagement Meeting. In support, the Nebraska Medical Association dedicated an issue of its Nebraska Medicine magazine to the National DPP and related work in Nebraska. Articles include “Early Intervention in Prediabetes, a New Frontier of Opportunity; National DPP in the Panhandle; Diabetes Prevention in the Rural Counties;” and “Nebraska Program Success Stories.”

National Diabetes Prevention Program

AMAPreventDiabetes – American Medical Association

This website offers a comprehensive assessment and guided process for health care organizations to implement a diabetes prevention strategy inclusive of an evidence-based National Diabetes Prevention Program lifestyle change program. AMAPreventDiabetes provides free access to best practice solutions, tools and resources, and profiles showcasing real-life experiences with diabetes prevention.

AADE Diabetes Prevention Program

The American Association of Diabetes Educators (AADE) funds 45 AADE-accredited and/or ADA-recognized diabetes self management education sites in 17 states to implement the National DPP.

Epidemiology and Evaluation Collaborative (EEC)

Stay up-to-date on the latest epidemiology and evaluation training and TA resources for 1305 and 1422. EEC is a group of state 1305/1422 epidemiologists and evaluators meeting on the second Wednesday of each month at 2:00pm ET.

National DPP Coverage Toolkit

This online toolkit was developed to provide information about the mechanics of covering the National Diabetes Prevention Program (National DPP) lifestyle change program, which is a year-long, evidence-based intervention program developed by the Centers for Disease Control and Prevention (CDC).

Ad Council Awareness Campaign Toolkit (2018)

A new phase of the National Ad Council’s prediabetes PSA awareness campaign toolkit was released in Nov 2018. NACDD encourages our Members and their partners to utilize these excellent copyright-free resources to raise awareness about prediabetes. The toolkit includes a customizable press release, high quality videos and graphics for use in social media, and PSAs for TV, radio and print. The toolkit also has materials you can use in your community – posters, postcards, social media graphics, newsletter language, and other PSAs that you can share. All media for the Ad Council campaign must be donated by the media outlet.

Planning Fundamentals

The Planning Fundamentals self-paced, online course provides a comprehensive overview of the process for developing and implementing a state health plan for chronic disease prevention and control.

Diabetes Council Advocacy Toolbox

The Advocacy Toolbox was developed by the Diabetes Council to provide resources for effective advocacy efforts and practical information to help with planning advocacy programs. Materials and resources can be applied to any chronic disease or policy development initiative.


Diabetes Self-Management Education

Diabetes self-management education (DSME) is the cornerstone of care for people with diabetes. This resource provides information on the definition of DSME, the National Standards for Diabetes Self-Management Education and Support, reimbursement, starting a community-based DSME program, and more.

Diabetes School Health Resource

This resource was developed by the Diabetes Council School Health Work Group and provides state and national resources on caring for students with diabetes in the school environment.

Working with Health Systems and Health Care Providers to Reduce Diabetes Complications

Too many people with diabetes do not receive recommended care and treatment. This NACDD series of fact sheets can help states and partners make the case for health system and provider practice redesign to improve care related to the four major diabetes complications. Using the Chronic Care Model as the framework, each fact sheet highlights data and research illustrating why change is needed and how change improves care.

Health Equity

NACDD is collaborating with the National Partnership for Action to End Health Disparities, a national initiative coordinated by the DHHS Office of Minority Health and the CDC to identify ways in which state health department chronic disease programs can better address health disparities and promote health equity through existing programs.
