The focus of the NACDD cardiovascular initiative is the Cardiovascular Health Network, comprised of health department program staff, epidemiologists, and evaluators working to improve diagnosis and management of hypertension through health systems.
The Network supports health departments in implementing population health improvement strategies in healthcare systems which are primarily associated with CDC’s Division for Heart Disease and Stroke Prevention-funded activities in the 1815 and 1817 grants. Major activities of the NACDD CVH Initiative include:

- Million Hearts® Initiative – NACDD’s project to support the national goal of preventing a million heart attacks by 2022
- CVH Epidemiology and Evaluation Projects – Epidemiology & Evaluation Collaborative, webinars and general guidance
- GIS Capacity Building for State & Local Health Departments – Training and resources for working with geographic information systems
- Advancing Team-Based Care – State teams are being trained on the use of collaborative practice agreements and the Pharmacists’ Patient Care Process to manage high blood pressure
