Cardiovascular Health (CVH) Council

The CVH Team provides peer-to-peer support to State Health Departments for cardiovascular disease prevention and management through the following initiatives:

  • CVH Council
  • CVH Area Network (CAN)

The CVH Council, with support from the NACDD CVH team, is a virtual network of all CDC-funded state and local partners and their contractors addressing cardiovascular disease prevention and management. 

The CVH Council supports training, identifies learning opportunities, and facilitates meaningful connections among state and local health departments. The Council also provides the cardiovascular health perspective to other NACDD councils, CDC leadership, and national partners.

CVH Council Leadership Team Overview

The CVH Council Leadership Team is made up of elected or appointed Chair and Liaison positions. These positions serve the CVH Council in the following ways:

  • Provide leadership and direction for the CVH Council and help set priorities for CVH Council offerings, peer engagement opportunities, and other support to health department colleagues.
  • Serve as the primary health department points of contact for the CVH Council, including representing the CVH Council to members, NACDD, CDC, and other national partners.
  • Represent the CVH Council at national in-person meetings and conferences, including CDC grantee meetings and NACDD meetings. 
  • Engage in decision-making that affects the strategic direction for the CVH Council; and
  • Participate in strategic planning with other members of the CVH Council leadership. 

CVH Council Leadership Team Members and Roles

As of September 1, 2023 Thelma Reis, MPH (AZ) has been serving as Chair of the CVH Council, prioritizing relevant communications (i.e., Off the Cuff, podcasts, etc.). 

   

Nicole Hamlet joined the CVH Council Leadership Team to help identify professional development opportunities (i.e. speakers, webinars, etc.)

Jim Peacock was the prior chair of the CVH Council Leadership team and continues to serve by identifying leadership opportunities (i.e, conferences, relevant comment periods, etc.) relevant to states.

Ashley Minaei works in collaboration with NACDD’s Health Equity Council to provide an equitable lens to the CVS Council.

CDC-Led Peer-to-Peer Engagement Calls

The CDC’s Division of Heart Disease and Stroke Prevention leadership and staff will facilitate calls to provide peer-to-peer opportunities for states to ask questions and share learnings with each other. For questions related to 23-0004, please reach out to your project officer. 

CVH Area Network 

NACDD launched the CVH Area Network (CAN) in January 2021 to build connections within eight geographic areas. Quarterly calls were held to facilitate sharing of promising ideas and state-developed resources among those working on heart disease and stroke prevention strategies. These calls happened quarterly and are led by peer liaisons to support sharing successes, opportunities, challenges, and resources, with an emphasis on program implementation. 
 

Evaluation Peer Network 

The Evaluation Peer Network (EPN) is comprised of evaluator and epidemiologists involved in programs funded by CDC’s Division for Heart Disease and Stroke Prevention and Division of Diabetes Translation. The EPN met bimonthly and discusses issues related to methodology, reporting, and dissemination of data and evaluation results.
