HOPE

In 2012, NACDD expanded upon its healthy community movement with its Healthy Opportunities through Prevention and Education (HOPE) project, a one-year project with financial support from CDC. The HOPE project aimed to continue working with, and provide technical assistance to, NACDDs existing healthy communities (ACHIEVE), train new communities nationwide on the five-phased healthy community change model, and communicate its healthy community experiences to interested members, national partners, and public health practitioners through the development of new tools, products, and publications.

In doing this, NACDD is proud to share more about its healthy community successes and make the case for why NACDD is a leader in healthy communities’ work through the following documents:

The Winter Park Health Foundation Case Study details the Winter Park Health Foundation’s ACHIEVE successes and challenges, and provides ample consideration for how health and community foundations can accelerate the healthy communities model to continue to make healthy choices the easy choices where people live, learn, work, play, pray and receive care.

The Healthy Communities Multisector Resource Compendium is a collection of more than 80 resources, tools, products, and publications documenting healthy community successes, lessons learned, and processes from around the nation. This document is intended for chronic disease prevention practitioners across any level who want to broaden their knowledge of how create a healthier community.

The Implications of Chronic Disease and How National Organizations Can Aid Local Prevention Efforts is a healthy communities white paper explaining the role that national organizations like NACDD can play in improving community health and preventing chronic diseases at the local community level.
