Root Causes of Health Initiative Case Stories

In 2019, NACDD collaborated with the Institute for Healthcare Improvement (IHI) to launch the Root Causes of Health Initiative (RoCHI). This initiative helped State Health Departments implement improvement plans to create meaningful change addressing the root causes of health in their states by applying IHI’s Psychology of Change Framework to Enhance and Sustain Improvement. This work is grounded in the STAR Framework. The below case stories describe how Michigan, Washington DC, and West Virginia built organizational capacity to address root causes of health in their states through their involvement in RoCHI.
