CHRONIC DISEASE SELF-MANAGEMENT PROGRAM

Chronic disease self-management programs (CDSMP) allow people with any chronic disease to participate and learn self-management skills. The program is a series of structured community based workshops or classes which hold participants accountable to goal setting. Participants learn about coping strategies, exercises, medication, communication skills, nutrition, decision making, and how to determine what approaches might be effective for them. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
