Approaches to Increasing Access to and Participation in Diabetes Self-Management Education This document tells the stories of five employers offering the National DPP’s lifestyle change program as a covered health or wellness benefit for their employees and discusses the varied approaches, challenges, and key factors contributing to their success.
Approaches to Promoting Referrals to Diabetes Self-Management Education and CDC-Recognized Diabetes Prevention Program Sites This report describes the approaches of four states and one city that have done substantial work to increase referrals to diabetes self-management education (DSME) programs and CDC- recognized diabetes prevention program sites operating under the framework of the National Diabetes Prevention Program (National DPP).
Diabetes Self-Management Education (DSME) Technical Assistance Guide , developed by the Centers for Disease Prevention and Control, identifies four key drivers that influence DSME access: 1) DSME programs, 2) payers and payment mechanisms, 3) referral policies and practices/health care systems, and 4) people with diabetes “willing to go” to DSME programs.
Diabetes Self-management Education and Support in Type 2 Diabetes: A Joint Position Statement of the American Diabetes Association, the American Association of Diabetes Educators, and the Academy of Nutrition and Dietetics AADE, ADA and AND, with assistance from the National Diabetes Education Program, issued this joint position statement in 2015 that defines the four times when a referral for diabetes self-management education and support is needed. These organizations also created a toolkit to accompany the joint position statement which is posted at the above link.
The National Diabetes Education Program (NDEP) works with partners to reduce the burden of diabetes and prediabetes by facilitating the adoption of proven approaches to prevent or delay the onset of type 2 diabetes and the complications of diabetes. NDEP and its diverse partners have developed educational messages, materials, and tools that are based on evidence from clinical trials.
