This issue brief highlights lessons learned and success stories shared by New York, Florida, and Montana to identify people with undiagnosed hypertension by searching electronic health records for patients who meet established clinical criteria; implementing a plan to diagnose and treat these patients; and calculating a health practice’s or system’s hypertension prevalence to compare it against local, state, and national data.
